Midwifery holds relevance across div erse settings\, as the proficiency of midwives plays a crucial role in the provision of care for women facing complications. In Low- and Middle-Inco me Countries (LMICs)\, such as Ghana\, a significant number of maternal an d neonatal deaths result from insufficient or delayed handling of preventa ble complications\, notably pre-eclampsia. Continuous training is particul arly essential for midwives in LMICs\, to effectively respond to the incre asing demand for specialised care in pre-eclampsia. As part of a mixed met hod evaluation of multi-level factors influencing midwives’ management of pre-eclampsia in Ghana\, we conducted a hospital-based quantitative cross- sectional study in 2021 to determine the pre-eclampsia specific training n eeds of midwives. Midwives completed an adapted version of the WHO Henness y-Hicks Training Needs Analysis questionnaire. Midwives required training in 14 priority areas mainly in the research and clinical domains. Training courses were identified as the preferred approach to address training nee ds and improve overall proficiency. Midwifery is an essential sustainable resource for improved pre-eclampsia outcomes in LMICs. The implementation of context-specific training\, integrating innovative and contemporary app roaches\, holds paramount significance in this regard.

Many midwives ent er the profession with a dream and internalised vision of the midwife they want to be\, how they will work with women\, families\, and other healthc are professionals. This vision becomes part of and adds to their midwifery identity. However\, within the profession\, there is often a dichotomy be tween the midwife you want to be and the increasingly challenging climate of midwifery. This can lead to midwives having to consider how they remain personally and professionally sustainable within the workforce. There is a growing body of evidence scrutinising the issue of retaining midwives in the workforce\, with several studies highlighting midwifery burnout\, exa cerbated by increasing workload\, bullying\, and financial constraints. Th emes from qualitative research conducted in New Zealand with employed and self-employed midwives will be presented. These included continuity of car e/partnership\, midwifery identity\, and career progression. One of the ma in themes that emerged was the notion of being an ‘ideal’ midwife who was able to remain personally and professionally sustainable by changing work settings. Abstract:

The prob lem: Obstetric violence creates visions of a brutal and purposeful assault \, however more covert practices cause similar psychological harm\, are le ss understood\, and more likely to be disregarded. Separating women from t heir well-baby at a caesarean section birth can cause long-lasting trauma. Method: A feminist phenomenological study\, using birthing theories to un derstand the experience of women separated from their baby at caesarean se ction birth without a medical indication. Results: We identified four main themes that were interlinked: Disconnection\, Emotional Turmoil\, Insight and Influence. This presentation expands on Influence and the sub-themes of power & control\, maternal choice & consent\, coercion and staff action s. While obstetric violence was perceived with the physically forceful act ions that some of these women experienced\, it also showed more insidious events which caused comparable traumatic responses. The term ‘obstetric ne glect’ was coined to symbolise maternity care where maternal choice and co nsent was voided by health care providers using power\, control and coerci on that influenced participant birth outcomes and experience. Conclusion: Birthing in an operating theatre environment limited the capacity of the p articipants to bodily autonomy and this vulnerability was not accounted fo r with woman-centred care. Midwifery training and values align with the ne ed for the protection of\, and advocacy for\, women. These findings demons trate the importance of a respectful\, empowering and supportive midwifery contribution through every pregnancy and birth\, in the development of po licies and procedures\, done in partnership with women.

Background: Teenage decision-making ability is a crucial stage of development. Marriage and adolescent pregnancy have an effect on decisi on-making ability. Inappropriate decision-making leads to the emergence of health problems\, delays in getting health services\, and an increased ri sk of complications during pregnancy and childbirth\, all of which have ne gative effects on maternal and neonatal health. Objective: This study aims to determine the decision-making experiences of adolescents who marry at a young age during pregnancy and childbirth. Method: This qualitative inve stigation employed a generic exploratory methodology. This study’s sample was selected using homogenous purposive sampling to include five pairs of adolescents who were married and had children younger than one year. The r esearch analysis was conducted according to the Collaizi step and analyzed using NVivo. Result: There are four major themes that emerge from the res earch findings: prior knowledge\, skills\, and preparedness to become pare nts\, decision making\, obstacles\, and expectations. Teenagers who marry at a young age and have children younger than one year lack the knowledge\ , skills\, and maturity to become parents\, and are therefore incapable of making decisions. Parents and spouses have a significant impact on decisi on-making because they are perceived to have superior knowledge\, skills\, and experience. Conclusion: During pregnancy and childbirth\, decision-ma king autonomy is influenced by a lack of parental knowledge\, skills\, and preparation. It is expected that training and education that involves fam ily decision-makers will increase the knowledge\, skills\, and readiness o f couples to become parents. It is also expected to increase participation .

An estimated 2 million babies are stillborn at or before the time o f their birth and 2 million babies die in the first month of life each yea r around the world. The International Stillbirth Alliance (ISA) is a globa l organization\, aimed at ending preventable stillbirths and neonatal deat hs via collaborations with researchers\, clinicians and parent advocates. As midwives are key providers of pregnancy and perinatal healthcare global ly there is the potential to advance the aims of ending preventable perina tal deaths and improving support and care of bereaved parents through coll aboration between ISA and the global midwifery community. To enable this I SA has launched its Health Provider Advocacy and Parent Advocacy toolkits which are readily adaptable to different country settings and easily utili sed by midwives globally. The Health Provider Advocacy toolkit provides in formation for midwives wanting to improve their skills in achieving deeper and more open communication with parents after a stillbirth by providing respectful bereavement care. The toolkit includes suggestions on ways to a pproach delivering the news\, acknowledging parents’ grief\, talking with parents about how and why their baby may have died\, making room for them to express their need for support\, and discussing a safe plan for future pregnancies. This presentation will provide an overview of the outputs dev eloped by the International Stillbirth Alliance to support healthcare prov iders including the toolkits\, the Global Guide for Stillbirth Advocacy an d Implementation supported by the Gates Foundation\, and resources availab le through our Western Pacific Regional Office the Stillbirth Centre of Re search Excellence.

Safe and high-quality maternity care is crucial for ensuring the b est possible health outcomes for both mothers and newborns (Renfrew et al. \, 2014). Unfortunately\, recent incidents of preventable harm taking plac e within maternity services have underscored the importance of whistleblow ing (Kirkup\, 2022\; Ockenden\, 2022). Whistleblowing involves reporting i ncidents of wrongdoing with the goal of putting an end to poor practices a nd reducing risk. However\, the act of whistleblowing may be met with sign ificant barriers and adversity for the individual blowing the whistle. Thi s presentation aims to discuss the findings of a scoping literature review that explores the experiences of health professionals\, including midwive s\, who find themselves in the position of whistleblowing. The presentatio n will explain the key issues\, barriers\, and challenges that current lit erature has reported regarding whistleblowing in maternity services. These include the structural power that enforces silence\, inadequate responses to whistleblowing\, and the mechanisms by which whistleblowing is suppres sed. It will also delve into the process of weighing up the personal risk to the whistleblower versus the moral obligation to protect mothers and in fants.

Abstract:

Purpose: Freebirth or unassisted birth is currently defined as t he deliberate choice to give birth without a regulated healthcare professi onal in countries where maternity care is available and easily accessible. Empirical evidence on this practice is limited and has mostly focused on understanding women’s motivations to freebirth as a matter of individual c hoice. Previous traumatic experiences\, alongside rejection of medicalised models of care and trust in the birth process have been identified as fac tors influencing women’s choice. Yet\, there is limited evidence on how wo men experience maternity care in the pregnancy leading to freebirth and th e impact this care has on their choice. Methods: A qualitative evidence sy nthesis was conducted to explore women’s perceptions of maternity care pro vision and their experiences of care when choosing to freebirth. 22 public ations between 2008 and 2023 and from ten different high-income countries were included. Thematic synthesis\, underpinned by a feminist standpoint\, was used to analyse the data. Results: Freebirth was rarely women’s prima ry choice. Faced with restrictive policies\, inequitable access\, and uneq ual power relationships with care providers\, women used peer support and engaged in extensive research to develop their self-care agency. Becoming midwives to themselves generated a sense of personal transformation and em powerment. Conclusion: When healthcare services fail to provide unrestrict ed\, equal and free access to a wide range of maternity care options\, wom en’s choice to freebirth becomes a matter of reproductive justice. Abstract:

Background: There is a major research gap relating to the impact of intravenous (IV) fluids administration during labour on maternal and neona tal outcomes. It is biologically plausible that a relationship between vol ume of IV fluids and primary postpartum haemorrhage (PPH) exists. Aim: To investigate the relationship between intrapartum IV fluids and PPH. Method s: A retrospective cohort study was conducted in a tertiary hospital from September 2021 to September 2022. Inclusion criteria were singleton pregna ncy\, planning a vaginal birth\, and admitted for labour and birth care be tween 37-42 weeks gestation. The study factor was IV fluids during labour. The primary outcome was primary PPH. Birth and postnatal data were obtain ed from the electronic medical records and paper fluid order documentation . Secondary outcomes included caesarean section and neonatal weight loss f ollowing birth. Results: 1023 participants were included of which 339 had a PPH (33.1%). Our main finding was that there was no association between high-volume IV fluids (≥2.5L) and PPH after adjusting for demographic and clinical factors (ORadj1.02 CI: 0.72\, 1.44). However\, there was a positi ve association between high-volume IV fluids and caesarean section (ORadj 1.99\; CI: 1.4\, 2.8) and neonatal weight loss (ORadj 1.8\; CI: 1.09\, 2.0 ). Conclusions: These findings are important to further knowledge relating to the administration of IV fluids in labour and the potential impact of this common practice. It identifies future research priorities around docu mentation of IV fluids and their relationship with pregnancy and perinatal outcomes.

Research Focus: Mothers’ experiences of childbearing during the Covid19 Pandemic (2020 to 2021) – A Narrative Exploration Qualitative Design: Using the Listening Guide Approach Created as a relational method of analysing qualitative data\, focusing on the in dividual voice (Gilligan et al\, 2003)\, the Listening Guide (LG) is a mea ns of ensuring that the voice of the under-represented is heard through re search. It is a way of focusing on the voices of women\, ensuring these ar e effectively represented in research (Petrovic et al\, 2015). Population and Sample New parents who were pregnant and gave birth during the Covid 1 9 pandemic March 2020 to August 2021. The sample is a convenience sample o f volunteers who agreed to take part. Total sample size is six. Data Colle ction Online /digital narratives of the birth experience sent in response to a series of social media posts across a range of media and networks. Ac counts were sent in via email following initial contact\, full information sharing and informed consent. Ethics: Ethical Approval granted by Swansea University College of Human and Health Sciences Research Ethics Committee Themes: 1. The service user voice “the world of mothering remained pretty isolating” 2. Themes relating to the experience which can inform practice and future service development\, including sustainability of midwifery se rvices. ” 3. Abstract:

Childbirth is a unique experience for women. In Ireland\, Major Obstetric Haemorrhage (MOH) is the most frequently reporte d severe maternal morbidity (SMM) with an incidence of 3.27 per 1\,000 mat ernities. Much is known now about the management of PPH and there is some research on women and their partner’s experience\, less is known about how the woman feels emotionally following a PPH\, or what informational needs and emotional support is required. The aim of this study was to understan d how women felt after experiencing a severe PPH\, to listen to their firs t-hand experience and learn what improvements could be made for future car e for women who experience a PPH. A descriptive\, quantitative approach wa s conducted using semi structured interviews with women who had a severe P PH of 2.5 litres or above within four and fourteen months postpartum. five women took part in this study. The women identified a lack of information provided to them about the reason for the significant bleed. The women vo iced they could overhear information about the event discussed between hea lth care professionals but not with the woman. The care the women received in the HDU was significantly different to the care they received on the p ostnatal wards\, and the women were not informed they were clinically well for transfer to the postnatal ward. It was reported the postnatal wards w ere busy and short staffed and the women looked for more emotional support from staff\, which was not available\, this had an effect on the postnata l wards.

Abstract:

Maternal and newb orn mortality rates in Papua New Guinea are among the highest in the world \, with around 170 deaths for every 100\, 000 women and 20 neonatal deaths per 1000 live births each year. The causes for many of these deaths are p reventable or treatable when women have equal access to safe\, quality hea lth care. Lack of midwives and resources increases the impact for remote c ommunities but also for those living in cities such as Port Moresby.

In April 2024 the fourth group of the PNG Midwifery Leadership Buddy Project met in Port Moresby with 14 PNG and 7 Australian midwifery mentors\, aiming to improve maternal and ne wborn outcomes in PNG. This twinning program of the PNG Midwifery Society and the Australian College of Midwives is funded by Rotary and informed by the ICM Member Association Capacity Assessment Tool (MACAT). PNG buddies identified quality improvement projects within their workplaces\, with the Australian midwives supporting the development of leadership and advocacy skills for the projects to be completed.

This presentation will introduce one of the 2024 buddy rel ationships and project which aimed to increase the known haemoglobin level of women accessing antenatal care in a clinic in Port Moresby. It will de scribe the importance of a known Hb and demonstrate the processes\, challe nges\, successes and setbacks experienced and highlight the importance of resilience and patience. It will emphasise how professional midwifery conn ections and mentorship can promote leadership\, improve maternity outcomes \, and strengthen the role of midwives across countries.

Abstract:

Occiput Posterior position is a variation of the norma l birthing process\, a complication of birth requiring patience\, interven tion\, or something in between. Occiput Posterior Position of the fetus is often referred to as “the most common” fetal malposition or as a variatio n of normal. As many as 30% of fetuses spend some time in this position du ring labor only 5-12% will remain so for the actual birth. Persistent Occi put Posterior Position may account for up to 18% of cesarean births as wel l as increased operative births\, along with maternal and neonatal morbidi ty and mortality. Anecdotally\, many birthing people are increasingly wor ried if their fetuses are posterior even at early gestations because of th e knowledge available in the media.

Midwives and accoucheurs have encountered this fetal position as documented into the Middle Ages (Louise Bouchard\, Jane Sharp). Current d ata and research and strategies the management of Occiput Posterior positi on present the intersection of the ART and SCIENCE of midwifery. However m ost current data is based in Eurocentric literature. Only recently has the re been literature from Spain\, China\, Japan\, and Iran to name a few mid wives from. Even these studies often reference the primarily English langu age literature aforementioned in their citations. Very rarely is there an article from a non-Eurocentric area where midwives and traditional birth a ttendants are prevalent.

T his presentation will focus on other perspectives on midwifery management of Occiput Posterior the non-primarily English-speaking voices that are em erging in literature focusing on this topic. The purpose is to highlight w hat strategies and options are available to those midwives providing care for birthing people particularly in those areas prone to disruptions in ca re due to conflict and disaster. What can we learn and how can we further support one another?

Dra wing from 15 years of experience as a seasoned midwife at Ghana’s largest tertiary health center\, the Korle Bu Teaching Hospital\, I will highlight the urgent need to address preeclampsia\, especially considering the limi ted time to achieve SDG 3.1\, having surpassed the halfway point. This dis cussion will delve into the challenges faced in LMICs\, particularly Ghana \, where preeclampsia emerged as the leading cause of maternal mortality i n 2023. I will share a poignant case study\, offering a firsthand account of the acute and often dire presentations of preeclampsia cases referred f rom the peripheries. Proposed initiatives will be explored\, including a d ata-driven approach to inform targeted interventions. Abstract:

I am Mim\, a Young Midwife Leader from Bangladesh. O ne year ago\, I assumed the role of supervisor of midwives on Basanchar\, a remote island near Hatiya Upazilla\, Bangladesh\, catering to 32\,574 pe ople and 7899 families\, many of whom were Rohingya refugees relocated fro m Coxs Bazar. The island faces numerous health challenges\, especially for women and children\, with limited access to medical facilities\, requirin g Navy Frigate transportation twice weekly. Upon arrival\, I encountered m idwives lacking confidence and support in their practice. As the first mid wife supervisor\, doubts surrounded my leadership abilities\, compounded b y the democratic system’s challenges. Despite skepticism\, I prioritized l istening\, reflection\, and evidence-based advocacy to empower midwives an d amplify their voices. Through collaborative efforts\, we transformed the team\, nurturing leadership skills and expanding their scope of practice. With a multidisciplinary approach\, we now offer comprehensive sexual and reproductive health services and manage most complications locally\, mini mizing the need for external transfers. Looking ahead\, our focus is on co mmunity awareness\, affirming the pivotal role of midwives\, and ensuring their practice aligns with standards. Continued education and research wil l further enhance our contributions to midwifery in Bangladesh\, fostering a culture of quality care and acceptance in Basanchar.

Midwives engage in p roactive preconception care\, assessing health factors and developing tail ored plans for subsequent pregnancies. Central to its mission is the enhancement of maternal health through t he promotion of Respectful Maternity Care (RMC) for all women throughout t he continuum of pregnancy\, childbirth\, and the postnatal period. Recogni sing the pivotal role of the relationship between pregnant women and their primary healthcare providers in shaping health outcomes for both mother a nd newborn\, WBFA highlights the significance of this connection in foster ing postnatal health and wellbeing.

Respectful maternity care is upheld as a fundamental human right\, deserving of every childbearing woman within every healthcar e system globally (WRA\, 2010). WBFA advocates for this principle\, emphas ising the inherent dignity and autonomy of women throughout the childbirth process. This ethos is echoed in the Lancet Midwifery Series (2014)\, whi ch asserts that midwifery is indispensable in addressing the challenges as sociated with delivering high-quality maternal and newborn care universall y. Moreover\, it is recognized as an effective strategy for promoting the health and wellbeing of women of reproductive age\, newborns\, and their f amilies\, thereby exerting a significant and sustainable impact on populat ion health outcomes. Through a grassroots and community-led approach\, Ma macare360 not only delivers essential maternal and child health services b ut also fosters a culture of empowerment and resilience among women.

Bangladesh is one of the mos t climate-change affected countries. Regular natural disasters\, extreme h eat\, air-pollution and outbreaks of vector-borne diseases put thousands o f pregnant women at risk\, particularly in hard-to-reach locations with li mited access to healthcare services. Midwives play a crucial role in addre ssing these challenges\; they are often the first point-of-contact for wom en in the healthcare settings and can orient women about birth preparednes s\, disaster preparedness and in general protecting their health from know n environmental and climate risks. The Government of Bangladesh has a work force of 7\,230 midwives. In 2023\, a training manual on the Climate-Chang e Impacts on Sexual and Reproductive Health in Bangladesh was developed by CCHPU and UNFPA then reviewed with national experts. Existing curriculum review found commendable inclusion of gender equality\, reproductive healt h and rights\, and disaster management but weak content on the intersectio n between climate change and health. A three-day Training of Trainers (ToT ) program was thus developed\, targeting 20 Nursing and Midwifery faculty\ , serving as a catalyst to disseminate essential knowledge and equip educa tors with the tools to integrate climate change perspectives into their te aching methodologies. A series of cascade training ensued\, with a day-lon g training for final-year Midwifery students and a specialized session for 50 practicing midwives across various health facilities. These efforts un derscore the government’s commitment to ensuring a health-workforce well-v ersed in addressing climate-change impacts\, particularly concerning sexua l and reproductive health. This content must now be integrated into nursin g and midwifery curricula across all education levels.

In this presentation\, new ideas for teaching suturing\, assessment and approximation of birth tears are presented. Learning the complex skill set for suturing birth tears takes time and lots of practice. Faculty and preceptors can support new learners with skill development activities and competencies that build these skills a little at a time from the ground u p. Objective assessment tools help learners know where they are in their l earning\, and aid faculty in identifying skills that need more work and pr oviding learners with activities to improve their skills. Becoming profici ent at suturing takes 3-7 years. While we want to prevent birth tears as m uch as possible\, let’s give new midwives the tools to keep refining their skills so every person who receives midwifery care gets the care they nee d and the best opportunity to heal well. Come listen to Nell’s story about new ways of teaching suturing developed after decades of working on these skills with midwives.

What do Uruguayan women perceive as Obstetric Violence? Do they question the role of informed consent\, agency or autonomy? Is le gislation enough to protect women at the time of childbirth? What answers do women who initiate a complaint seek? What can midwifery improve or cont ribute to prevent these experiences? Why is a feminist and reproductive ju stice vision necessary in the monitoring of these cases? These are some of the questions that I raise and whose answers I will try to give\, based o n the analysis of 35 cases of women at the stage of reporting obstetric vi olence in Uruguay. It is a view that starts from legal practice\, as a law yer who accompanies and advises these complaint processes.

Moxibustion refers to the practice of burning the herb “moxa”- mugwort o r Artemistae argyi – for its health properties. This practice originated f rom traditional Chinese medicine. Its most common use in childbearing popu lations is to assist in turning a breech presenting fetus to vertex prior to labor. This practice has an abundance of evidence attesting to its effe ctiveness with limited or no harm\, and yet it is not often used\, in part \, due to providers’ lack of understanding. Hypnobirthing is a set of tech niques that aim to give women and birthing people a more serene and joyful birthing experience by minimizing the pain and stress commonly associated with labor. Hypnobirthing involves training related to physiologic birth processes\, relaxation techniques\, self-hypnosis\, visualization\, light touch\, and breathwork. It is taught in classes or workshops and can be su pplemented with audio recordings and reading materials. It does not guaran tee a completely pain-free labor\, however\, studies show that many women who use hypnobirthing techniques report having a less painful and more pos itive birthing experience. Sustaining midwifery mastery of alternative and complementary therapies that provide options to our patients requires edu cation and support. Two Georgetown midwifery students will share the lates t information about these therapies in the 9th annual Georgetown Universit y Student Café at the VIDM. Integrative therapies offer a more wholistic a pproach to health care that combines techniques from several disciplines t o be used in lieu of or in conjunction with medical treatments depending o n the conditions and patient preference.

\n< p>Background: Midwives are the foundation to reducing maternal and neonatal mortality. Midwives facilitate physiolo gical birth processes to support and enable women to have an optimal birth . Maternity care in the Czech Republic is obstetric led\, medicalised and patriarchal. The Midwifery Unit Network collaborated with a Czech hospital to provide training for midwives on optimal birth techniques as part of a wider initiative to develop the first midwife-led unit in Prague. Aim: To explore the facilitators and barriers for implementing the learning objec tives of a bespoke ‘Optimal Birth’ Programme into practice in a state hosp ital in Prague\, Czech Republic. Method: A qualitative descriptive phenome nological approach was followed. Purposive sampling was used to select nin e participants who met the selection criteria\, were willing to participat e\, and signed the consent form. Data collection was via two participation observations and semi-structured interviews. Thematic analysis was carrie d out on the data retrieved. Results: Six main themes emerged: politics an d society\, organisational culture\, supporting physiology\, models of car e and technology use\, resources to support practice shift and supporting the practice shift. Discussion: Many facilitators and barriers were discus sed\, ranging from the political and organisational level to the funding a nd resources available and the confidence of the individual practitioners.

Skilled birth attendants\, particularl y midwives\, play a crucial role in reducing preventable maternal and newb orn deaths. In January 2024\, Helping Children Worldwide collaborated with six other organizations to host a 5-day training conference on emergency procedures in maternal and newborn health in Sierra Leone. Our presentatio n will argue that organizations should prioritize investments in training and supporting midwives in LMICs\, highlighting the positive and sustainab le impact on both health outcomes and broader development goals. Further\, presenters will talk in a panel discussion about best practices for these trainings\, highlighting practical steps to planning a training conferenc e\, lessons learned\, and the value that collaboration brings to this work .

Background: The role of the mid wife is emotionally demanding with many midwives experiencing high levels of stress and burnout\, and a great number considering leaving the profess ion. This has serious implications for the delivery of high-quality\, safe maternity care. One of the major factors leading to job dissatisfaction i s the conflict between midwives’ aspiration of truly ‘being’ with the woma n and the institutional expectations of the role which focuses on the ‘doi ng’ aspects of the job. ‘Being’ present to a woman’s psychological needs\, whilst meeting the institutional demands\, requires high levels of emotio nal intelligence (EI) in the midwife. Therefore\, enhancing midwives’ EI c ould be beneficial. EI education programme: An EI programme was made avail able to midwives with the intention to promote their emotional intelligenc e and enable them to utilise relaxation techniques for those in their care . Aim: To explore midwives’ perspectives on the influence of the EI educat ion programme on their emotional wellbeing and experiences of practice. Me thod: The study took a descriptive qualitative approach. Thirteen midwives participated in focus group interviews. The data were analysed using them atic analysis. Findings: The overarching theme of ‘The Ripple Effect’ incl uded three themes of ‘Me and my relationships’\, ‘A different approach to practice’ and ‘Confidence and empowerment’. The programme was seen to crea te a positive ripple effect\, influencing midwives personally\, their appr oach to practice\, and feelings of confidence in their role. Conclusion: E I education can reduce emotional stress in midwives\, enhance their empath y and feelings of confidence\, thus\, improving the quality of care.

\nThis presentation by ‘Remi Ejiwunmi\, RM MSc\, Chair of the Association of Ontario Midwives ’ Midwifery Sustainability Project Steering Committee\, and Elizabeth Bran deis\, RM MSc\, Staff Lead of the Midwifery Sustainability Project\, will describe the trends and conditions in Ontario midwifery that are leading t o unprecedented rates of burnout and attrition from the profession. The pr esentation will provide a brief overview of Ontario midwifery within a pub licly funded model in which midwives work mainly in community-based\, midw ife-led practice groups and provide choice of birthplace including home an d hospital birth\, and in some communities\, the option of free-standing b irth centres. It will then explore the current state of midwifery and situ ate it within an evidence-based framework of the systemic factors that act as enablers or barriers to midwifery sustainability. The presentation wil l include a description of the first phase of the Midwifery Sustainability Project\, the focus of which has been to understand the scope of the issu e of sustainability for midwives. The presenters will then describe how th e project will move from research to action to address the barriers to mid wifery sustainability in Ontario.

Abstract: The ut erus is a wonder of bioengineering. If only we could respect its mechanism better\, we could encourage more babies to be born into the hands of a mi dwife\, who can then swiftly transfer them into the arms of their mother. Within the space of a few hours\, the uterus is transformed from a safe ha ven\, a strong fortress for a vulnerable fetus – into the means of their t ransition to the outside world. Since the mid twentieth century\, birth wo rkers have been given a one dimensional view of uterine activity – that co ntractions start from the fundus and push downwards. Managing labour using this flawed model has tethered women to the obstetric bed\, encouraged ov eruse of synthetic oxytocin and denied the uterus its full directive power . It’s time to think again. We need to learn to respect the uterus\, to re spect its owner and to respect its occupant\, the baby\, who also plays an active part in birth. Fortunately\, we can rehumanise childbirth with a f ar more accurate user-friendly model – The Womb’s a Balloon.

Childbirth and midwifery have consistently belonged to and been car ried out by women\, for women\, among women. Anthropological studies posit midwifery as the world’s oldest profession\, with midwives\, nurses\, and healers often facing persecution together. The lack of coincidence in the se events lies in the fact that women’s autonomy\, freedom\, and sexuality have perpetually been perceived as threats by those seeking control. Huma n childbirth behavior stands in contrast to other primates\, as human moth ers actively seek assistance during childbirth\, primarily from another wo man who can provide support and share experiences acquired from predecesso rs or previous births. Despite centuries of evolution\, midwives continue to play a crucial role in human survival and women’s health. The World Hea lth Organization (WHO) includes the care of girls\, adolescents\, and adul t women throughout the pregnancy continuum in its responsibilities. The pr esence of midwives in perinatal care has proven to significantly reduce ra tes of cesarean sections and maternal and neonatal morbidity and mortality . The professionalization and adaptation of midwives to the changing needs of diverse populations have occurred over history. Despite being historic ally expelled\, prohibited\, belittled\, insulted\, devalued\, and even pe rsecuted to the point of extermination during the inquisition\, midwives c urrently advocate both nationally and internationally for the recognition of their profession and autonomy. Organizations like the WHO\, ICM\, and R ELACAHUPAN have underscored the importance of midwives’ autonomy. Updating knowledge about the historical struggle for midwives’ autonomy is crucial to grasp the significance of protecting the profession\, benefiting both midwives and the women’s sexual health.

An exploratory assessment of a global health partnership (GHP) for midwive s between the UK and India. India has recently embarked on a transformativ e maternal health program to strengthen midwifery training across the coun try. UK midwives from a South East NHS Trust were partnered with midwives in Hyderabad\, India in a project designed to strengthen midwifery leaders hip. Education resources were provided via online systems and a clinical s ite visit was undertaken. Aim To explore Indian and UK midwives’ experienc es of participating in a global health partnership including what were the reciprocal perceived benefits and challenges in knowledge and leadership capability and development for both groups of midwives participating in th e project. Method A qualitative exploratory study using semi-structured qu alitative interviews to provide insight into participants’ experience of p articipating in the GHP following the conclusion of the project. Participa nts consisted of 14 UK and Indian midwives. Results Midwives in all settin gs found the partnership beneficial. Participants shared enhanced leadersh ip capacity including examples of compassionate leadership and emotional i ntelligence. The GHP had provided a renewed sense of worth and encouraged continuing advocacy in a challenging environment Conclusion Midwives in al l settings demonstrated enhanced leadership skills. The UK midwives report ed increased joy and satisfaction in their UK roles. The research findings suggest GHP opportunities for UK midwives may offer leadership growth and retention opportunities for the NHS.

Bringing the practices of mindfulness to our patients and ourselves can significantly impact our patients’ relationshi p to pain and fear in labor\, birth\, and life. In this presentation\, par ticipants will have an opportunity to experience a mindfulness practice an d learn ways to implement mindfulness in midwifery\, including for childbi rth and parenting. Participants will be exposed to how mindfulness meditat ion can decrease stress during pregnancy and beyond and hear about mindful ness skills for working through pain and fear in childbirth. Further\, par ticipants will learn how to encourage mindfulness life skills for parentin g with wisdom\, kindness\, and connection from the moments of birth\, as w ell as how mindfulness skills may be implemented as a way to disrupt inter generational patterns of suffering. In particular\, this presentation will offer concrete ways to bring mindfulness to the contractions of labor\, a nd to the space in between the contractions of labor. The potential for se parating “pain” from “suffering” using mindfulness practices will be explo red\, which can be applied to labor\, and of course\, to life. We will exa mine the research around mindfulness-based interventions\, the relationshi p between perinatal stress and outcomes\, and the potential that mindfulne ss strategies have for reducing health disparities.

Background: Counter-hegemonic practices among midwives as a way to carry out their work from a situated position concerned about socio-cultural cha nges carries a strong social and political commitment. Reaching this point is not an easy road\, hence the importance of knowing and analyzing the c ounter-hegemonic processes of midwifery and its impact on the profession i n Chile. Method: Qualitative design within the paradigm of descolonial fem inist epistemologies\; theoretical/practical/methodological proposal which works with the body\, subjectivity\, and the territory. Three body maps w ere developed by the midwives who also acted as authors of this study\, an d were analyzed via image analysis. Results: The body maps and their narra tives indicate processes\, stages\, people\, and key scenarios which let t he midwives learn various counter-hegemonic ways to exercise their profess ion. These processes were not always linear\, and we can also observe a co ntinual questioning of traditional\, sometimes violent forms\, of carrying out midwifery. Discussion: The emergence of critical reflections around t he exercise and tasks of midwifery is a phenomenon which has drawn increas ing attention. In this sense\, the counter-hegemonic practices of Chilean midwives and the critical reflections from midwives in Europe and the Angl o-Saxion nations have important and necessary encounter points\, key eleme nts to move towards a midwifery which is concerned with people and their e xperiences\, with greater meaning and socio-political commitment.\n

< span style='font-weight: 400\;'>In recent years\, the integration of light therapy into healthcare has emerged as a groundbreaking innovation. This novel approach\, also known as photobiomodulation (PBM\,Red Light Therapy\ , LLLT) harnesses the therapeutic potential of specific wavelengths of lig ht (Red\, Blue and Near Infrared) to accelerate wound healing\, reduce the risk of infections and enhance overall well-being for mothers. The benefi ts of light therapy in postpartum care are multifaceted. PBM has been show n to alleviate acute pain by activating TGF-B1 and TRPV 1 in the cells pro moting a more comfortable recovery without medication. The non-invasive\, quick acting\, harmless nature of light therapy makes it an important opti on for mothers seeking alternative methods to manage pain without relying solely on pharmaceutical interventions. Light therapy stimulates cellular repair and regeneration\, expediting the healing of perineal tissues/cesar ean incision\, reducing inflammation and supports faster healing. This acc elerates recovery from labor and birth contributing to a quicker return to normal functionality and comfort. PBM increases cellular energy (ATP) thr ough the mitrochondria in our cells therefore reducing postpartum depletio n. This abstract briefly explores the transformative potential of light th erapy in postpartum care\, emphasizing its holistic approach to recovery.< /span>

The Birth Future International Project explores innovative future scenarios for birth care\ , grounded in developments shaped by birthing communities and midwives as key agents of change. This study employs a qualitative method approach\, i ntegrating traditional participatory research methodologies with innovativ e\, arts-based practices to guide participants through immersive processes of exploration and co-creation. Our cross-cultural sampling includes midw ives from the Netherlands (#) and Peru 90 (30)\, alongside service users ( #) from India\, offering diverse perspectives on the evolving dynamics of birth care.
\n
Analysis: Our data analysis is based on action research analysis. Preliminary data analy sis has informed the development of a zine\, which synthesizes participant contributions into an accessible\, creative format. This zine unveils a t ransformative vision for birth care\, such as different ways of developing perinatal technology (technology for autonomy\, rather than control)\, th e role of the community in which birth (care) is embedded\, addressing sys temic injustice and inequality in global north and south\, midwives as edu cated birth advocates/portals for bridging new ways of (birth) care\, thos e articulated through critical reflections on participantsâ€™ lived experi ences and current practices. Participants then identified innovative\, com munity-centered care models that emphasize equity\, cultural responsivenes s\, and inclusivity. Furthermore\, the preliminary findings underscore the potential of participatory\, arts-based research to amplify underrepresen ted voices and foster critical discourse on the future of maternal and new born care. By integrating interdisciplinary methodologies and global persp ectives\, this study contributes to the growing body of literature on the co-creation of equitable\, humane\, and sustainable birth care practices.

Emerging from the 2014 Lancet Series on Midwi fery\, the Alliance recognizes that research inequities persist globally\, with dominant narratives often excluding critical perspectives from diver se practitioners and communities.

QMNC’s collaborative approach deliberately centers LMIC voices\, particularly emphasizing the integral role of midwifery knowledge. By fost ering international partnerships across researchers\, clinicians\, advocat es\, and policymakers\, the Alliance creates pathways for equitable partic ipation in knowledge creation and dissemination. This model challenges tra ditional hierarchies that have historically privileged certain forms of ex pertise while dismissing others.

\n< p>In this presentation\, Lynnelle will sh are preliminary findings from her Grounded Theory study. How do profession al connections and relationships impact midwives wellbeing and career sust ainability?
\n
Emerging findi ngs highlight the importance\, value and protective nature of professional connections for midwives\, as well as the vulnerabilities that form in th eir absence. The data demonstrates that regardless of career stage\, the s trength of midwives relationships and connections is intrinsic to their su stainability and wellbeing.
\n
Glaserian Grounded Theory was used in this study. In-depth interviews we re conducted with 29 Midwives from across Australia to capture a depth of understanding of how collegial relationships impact across the career cont inuum.
\n
Participants descri be how the absence of connection in midwifery is taking a personal and pro fessional toll\, resulting in psychological and physical impacts such as f eelings of disconnection\, disillusionment\, isolation\, fear and dread\, all of which have a strong correlation with absenteeism\, reduction of shi fts\, and attrition.
\n
In co ntrast\, participants who experience a practice environment that supports and values professional connections and relationships reported high levels of professional satisfaction\, collaborative interdisciplinary teams\, ro le understanding\, autonomy\, trust and support to practice to their full midwifery scope\, and to continue to develop professionally and explore di verse career paths in midwifery.
\n
Conclusively\, participants viewed relationships and connections as vital to their sustainability in midwifery and to their psychological and physical wellbeing. These emerging findings point to an urgent need to re cognise and legitimise the importance of connection and professional relat ionships for midwives to sustain and thrive within the profession.< /p>\n

Background: Adolescent pre gnancy is a global health concern that affects the well-being of both moth ers and newborns. Adolescent pregnancy is linked to risk factors that cont ribute to adverse pregnancy and perinatal outcomes.
\n
Objective: This review aims to ascertain the fac tors contributing to adverse adolescent pregnancy outcomes.
\n
Method: The primary databases for the re view were PubMed\, Google Scholar\, and Science Direct. From 2020 to 2024\ , 1103 papers were identified\, of which 121 were chosen for full-text rev iew after thoroughly screening and eliminating duplicates. Fifteen studies were included in the review.
\n
Result: An analysis of fifteen articles indicated that adolescent preg nancy is substantially associated with adverse pregnancy outcomes\, affect ed by factors including inadequate education\, psychological and social pr essures\, low socioeconomic status\, poor nutritional health\, harmful beh aviors during pregnancy\, an unfavorable social environment\, and insuffic ient antenatal care. Improving primary health services for adolescent preg nancy is essential\, including the delivery of early sexual education and contraception options.
\n
Con clusion: Adolescent pregnancy is often associated with adverse pregnancy o utcomes. Improving the use of adolescent maternal health services\, along with the execution of focused sex education programs and contraceptive met hods in families\, educational institutions\, and primary health care cent ers\, is essential for reducing the incidence of adolescent pregnancies.

Key message: Adolescent pre gnancy is associated with adverse pregnancy outcomes. By identifying the c haracteristics that lead to adverse pregnancy outcomes in teenagers\, it i s anticipated that health education can be delivered to both adolescents a nd their familial or social contexts to avert these problems.

Background: Midwifery students juggle academic p ressures\, personal obligations\, and the transition to clinical practice\ , often leading to burnout and stress. Activities that build resilience ar e beneficial in facilitating student engagement and promoting emotional we llbeing.
\n
Objectives: This study evaluates the effectiveness of Student Midwife Support Circles (SMSC ) as a co-curricular peer support activity\, in building a sense of belong ing and professional identity\, and developing emotional wellbeing.
\n
Methods: Participants were recru ited from past and current midwifery students at a regional Australian uni versity. A mixed-method approach used a cross-sectional online survey and individual interviews. Quantitative data (n=31) was reported as descriptiv es and compared based on attendance at the SMSCs. Qualitative data from bo th the survey (n=31) and interviews (n=3) was analysed using thematic anal ysis.
\n
Results: Compared to students (n=9) who had not attended SMSCs\, students who attended any num ber of SMSCs (n=22) reported greater compassion satisfaction and slightly higher self-efficacy. In addition\, attendance significantly improved stud ents’ views that support groups reduced stress whilst on placements\, impr oved belonging to the profession and in becoming a better midwife\, and in creased student well-being. The thematic analysis confirmed these results\ , and the value of an academic run the support group.
\n
Conclusions: Students who have been to any num ber of SMSCs have much more favourable views of the usefulness of these gr oups and there is some improvement in the satisfaction of care they provid e and their confidence in dealing with challenges.

Metodos: Se r ealizaun estudio observacional analitico con mujeres que aceptaron un met todo anticonceptivo inmediatamente despues de un aborto en un hospital pub lico peruano (N = 399). De ellas\, se contactó telefónicamente a 175 parti cipantes seis meses después.

Resultados: Solo el 39\,42% de las mujeres mantuvo el uso del método anticonceptivo. Se hallo una asociac ion significativa entre la edad (p = 0\,049) y la nacionalidad venezolana (p = 0\,043) con el uso de metodos anticonceptivos seis meses despues del aborto.

Objective : To determine whether nationality and age are associated with the use of prescribed contraceptives in the post-abortion period\, after a six-month follow-up.
\n
Methods: An ana lytical observational study was conducted with women who accepted a contra ceptive method immediately after an abortion in a Peruvian public hospital (N = 399). Of these\, 175 participants were contacted by telephone six mo nths later.
\n
Results: Only 39.42% of the women continued using the contraceptive method. A significan t association was found between age (p = 0.049) and Venezuelan nationality (p = 0.043) with the use of contraceptive methods six months after the ab ortion.

Background: A migrant is a person who moves from one place to another\, especially to find work or better living conditions . Some migrants come from refugee backgrounds with well-founded fear of be ing persecuted for reasons of race\, religion\, nationality\, membership o f a particular social group or political opinion. From 2019 to 2021\, over 47% of women who accessed maternity care through Western Health were born in a country other than Australia\, with over 60 languages identified as their first language.
\n
Aim: To explore the culturally specifi c maternity care experiences of migrant women and to consider ways to impr ove care.
\n
Methods: Four focus group discussions were conduct ed with women of three language groups: Oromo\, Dinka\, and Arabic. The fo cus groups were co facilitated by the research team and a bi-cultural work er\, with an interpreter in attendance. Discussions were audio recorded\, professionally transcribed and thematically analysed.
\n
Results: Thirty-one women participated. All had at least one pregnancy in Austra lia. Key themes included communication (and miscommunication)\, cultural m isconceptions\, racism\, loss of culture\, and desire for continuity of ca re. All the women found maternity care in Australia generally satisfying a nd respectful\; however\, they offered some tangible ideas for improvement .
\n
Discussion: Addressing cultural misconceptions and combati ng racism within healthcare settings is imperative to create a welcoming a nd inclusive environment for migrant women. Effective communication\, acco mmodating linguistic diversity\, and cultural nuances need to be prioritis ed. This includes providing interpreter services when needed and employing culturally competent staff who understand the traditions\, beliefs\, and practices of diverse communities.

Background: The nutrition of a newborn during the initial six mo nths is crucial\, and exclusive breastfeeding is advised to fulfill these requirements. Nonetheless\, numerous variables\, including employed mother s\, sometimes obstruct attaining exclusive breastfeeding objectives. The W HO established Global Nutrition Targets 2025 aiming for at least 50% of ne wborns to be exclusively breastfed\; however\, current data indicates that some regions\, including Indonesia\, have failed to achieve these targets .
\n
Case report: Our presen tation examines the continuity of care activities of a 28-year-old woman\, a working mother who dedicated herself to exclusive breastfeeding upon re turning to work. The care encompasses health promotion\, the advantages of exclusive breastfeeding\, nutrition to enhance breast milk production\, e ngagement of support networks\, preparation of exclusive breastfeeding man agement\, and oxytocin massage techniques. There were factors influencing the success of exclusive breastfeeding\, such as the hospital policy forbi dding formula milk distribution to newborns. This policy relies on the adh erence of healthcare professionals who encourage and motivate mothers to m aintain exclusive breastfeeding without introducing formula milk. This sup port is provided during pregnancy and extends into the postpartum period\, with the expectation that continuity of care initiatives will significant ly benefit aided mothers and enhance the effectiveness of exclusive breast feeding among working women.
\n
Conclusion: Ongoing support from family and continuity of care from he alth professionals are crucial in enabling working women to breastfeed exc lusively. Engagement of family members and modifications to workplace poli cies can enhance the likelihood of successful exclusive breastfeeding.

Methods: This study examined 21 peer-reviewed articles sourced from PubMed\, ScienceDirect\, and Google S cholar\, following PRISMA-ScR guidelines. The examined factors included ma ternal age\, prior vaginal birth experience\, cervical dilation\, access t o healthcare\, and psychosocial support.
\n
Results: A successful VBA C is often associated with younger maternal age\, a prior history of vagin al birth\, spontaneous onset of labor\, and adequate cervical dilation at the time of admission. Psychosocial support\, encompassing familial encour agement and guidance from healthcare professionals\, significantly influen ced decisions regarding VBAC. Hospital policies supporting VBAC and improv ing access to emergency services have increased success rates.
\n
Con clusion: The success of VBAC is determined by multiple medical\, social\, and institutional factors. Our review underscores the necessity for standa rdized practices that facilitate VBAC and enhanced counseling to empower w omen’s decisions regarding childbirth. Future research should investigate large\, diverse\, multicenter cohorts to validate the predictors of VBAC s uccess and assess long-term outcomes compared to repeat cesarean sections.
\n
Key message: Empowering natural birth after a cesarean requires a multifaceted approach\, with a particular focus on medical factors. Equal ly important are continuous care and fostering a positive mindset\, which is essential to achieving a successful natural birth and is supported by m idwives\, obstetricians\, and all birth workers.

Back ground: Pelvic organ prolapse (POP) is a common condition that can si gnificantly impact a woman’s quality of life. Pelvic floor muscle training (PFMT) is recommended as a first-line conservative treatment for prolapse \, but evidence from low-resource settings is limited.
\n
Objectives: This study aimed to assess midwif e-led pelvic floor muscle training on prolapse symptoms and health-related quality of life (HRQoL) among women with mild to moderate prolapse in Eth iopia\, 2024.
\n
Methods: A c ommunity-based\, parallel\, two-arm cluster randomized controlled trial wa s conducted in Dale and Wonsho districts of Sidama Region\, Ethiopia. Wome n with symptomatic POP stage I-III were randomized by cluster to receive e ither midwife-led PFMT plus lifestyle counseling (intervention group) or l ifestyle counseling alone (control group). The primary outcomes were chang e in prolapse symptom score (POP-SS) and prolapse quality of life (P-QoL). Mixed-effects generalized linear model was used to determine the effect s ize at 99% confidence level.
\n
Results: A total of 187 women were randomized (intervention = 89 and co ntrol = 98). At sixth month\, the intervention group showed significantly greater improvements with a mean change difference of: -4.1 (99% CI: -5.38 \, -2.83) in prolapse symptoms\; -11.48 (99% CI: -15.9\, -7.1) in physical domain\, -12.65 (99% CI: -19.3\, -6.1) in psychological domain and -9.47 (99% CI: -15.5\, -3.5) in personal relationship domain of P-QoL. A signifi cantly higher number of women in the intervention group perceived their co ndition as ‘better’ after the intervention. Women with earlier stages of p rolapse (stage I and II) experienced higher benefits compared to stage III .
\n
Conclusions: A midwife-l ed PFMT combined with lifestyle counseling significantly improves prolapse symptoms and quality of life in mild to moderate POP. This strategy can b e integrated into the existing maternal and reproductive health programs t o address POP in low-income settings where access to trained specialist is limited.

Background: Maternal mental health disorders\, especiall y depression and anxiety\, correlate with adverse pregnancy outcomes such as preterm birth\, low birth weight\, and postpartum depression\; however\ , the role of psychosocial variables in alleviating these risks remains in adequately understood. These concerns are frequently overlooked during pre natal care\, particularly in resource-limited environments.

Object ive: This scoping review seeks to examine the influence of maternal mental health disorders on pregnancy outcomes and to identify psychosocial facto rs that may mitigate associated risks.
\n
Methods: A scoping review w as performed utilizing resources such as PubMed\, Google Scholar\, and Sci enceDirect. According to the established inclusion criteria\, fifteen stud ies were selected from an original pool of 831 publications published betw een 2019 and 2024.
\n
Results: Our review demonstrates a significant correlation between maternal depression and anxiety with preterm birth\, i ntrauterine growth restriction\, low birth weight\, and cesarean delivery. Psychosocial factors\, including partner support and stress-coping strate gies\, significantly mitigate these risks. Timely identification and thoro ugh prenatal mental health therapies are essential for enhancing long-term outcomes for moms and infants.

Conclusions: The mental health of the mother profoundly affects pregnancy outcomes. Incorporating mental hea lth assessments and psychosocial assistance into standard prenatal care is crucial for mitigating adverse effects. Future research should concentrat e on creating scalable care customized for various populations and providi ng continuous care during the perinatal period.

Background: During pregnancy and early parenting\ , autistic adults contend with increased sensory demands and services ill- suited for their needs. How Aotearo’as unique maternity system fares with this cohort is unknown due to the dearth of local data.
\n
Methods: 15 autistic people who gave birth i n New Zealand between 2012-2022 were recruited through social media. Parti cipants predominantly identified as female but represented varied backgrou nds and childbearing experiences. Using Kathy Charmaz’s constructivist gro unded theory\, verbal and written semi-structured interviews were conducte d by the lead author\, an autistic midwife-researcher\, utilising inclusiv e practices.
\n
Results: Auti stic birthing experience in Aotearoa shares many of the same hallmarks fou nd in international research\, including heightened sensory sensitivities and late diagnosis. Participants reported varied benefits from continuity of care models but also demonstrated significant self-determination in nav igating childbearing by prioritising their physical health\, mental well-b eing\, or the needs of others during decision-making and support-seeking.< /span>
\n
Conclusions: This study ha s highlighted various challenges autistic parents negotiate during childbe aring and the transition to parenthood. Midwifery-led\, continuity-of-care models can ameliorate some challenges\, but decision-making was chiefly i nformed by the participant’s awareness of their individual physical\, ment al\, and domestic needs. The findings will assist in the provision of enab ling and empowering care to autistic clients and can improve the morbidity and mortality rates seen in the broader autistic population.

Induction of labour is increasing worldwide \, and acupressure wi th its holistic approach to care offers a potential opportunity to reduce the medicalisation of childbirth. Practising acupressure would allow midwi ves to provide better care to women\, enabling them to experience a safer and a more natural childbirth .Acupressure is an ancient Chinese technique based on the principle of acupuncture which involves the use of finger pr essure (without the needle) on acupuncture points\, situated on the meridi ans. The acupressure points most commonly used for induction of labour hav e a very strong effect on blood flow to the uterus \,promoting cervical di latation\, increasing the release of prostaglandin and oxytocin facilitate the production of endorphins\, helping the mother to relax\, promoting ph ysical and emotional wellbeing.
\n
Acupressure for induction of labour should be an option available to women in childbirth as it would allow them to regain some control of thei r birth experience \,as well as facilitating a more natural and less medic alized childbirth.
\n
Acupres sure is a non-invasive technique that can be easily applied by a birthing partner and/or midwives\, promoting birthing partner involvement. It is co nsidered comfortable\, safe and non-painful\, potentially reducing the nee d for pharmacological induction methods. Acupressure can offer increased c hoice and control and is argued to increase the chances of achieving a phy siological birth. This approach may also reduce the cascade of interventio n and reduce healthcare costs.

\n< p>Background: Stillbirth bereavement care is increasingly recognised as essential. Despite guidelines for stillbirt h care\, gaps remain in bereaved parents experiences of compassionate care . Inadequate education in healthcare curricula may contribute to negative experiences for both families and providers. This survey assesses whether and how healthcare curricula prepare midwives to care for families experie ncing stillbirth.
\n
Methods: An international cross-sectional survey\, co-designed with subject matter experts\, explored midwifery students and recent graduates experiences. O utcomes included educational content and confidence in knowledge of bereav ement care\, measured from 1.0 (Not confident at all) to 5.0 (Extremely co nfident). Quantitative and qualitative analyses were performed.
\n
Results: Of 127 responses from 14 co untries\, 87% reported receiving some level of stillbirth education. While 92% had received teaching on stillbirth definitions\, only 50% had bereav ement care training\, and just 43% received education on investigations fo r stillbirth causes. Confidence was highest for defining stillbirth (4.0) but lower for bereavement care (2.6) and investigations (2.6). Only 42% fe lt prepared to support families\, with 86% expressing a need for more clin ical experience and 50% wanting more support in accessing psychological se rvices for themselves. Qualitative analysis revealed four themes\, includi ng gaps in structured education\, disconnect between theoretical and pract ical knowledge\, workplace support deficits\, and system-level barriers to holistic care.
\n
Conclusion s: Significant gaps exist in midwifery bereavement education. Strengthenin g curricula with structured training\, greater clinical exposure\, and wor kplace support may help in improving care and reducing provider distress. Urgent action is needed to equip midwives with the skills and confidence t o support bereaved families.

A review of clinical data\, community health reports\, an d maternal health education programs highlights key misconceptions\, inclu ding: The belief that jaundice is harmless and does not require medical at tention. The misconception that sunlight exposure alone can cure jaundice\ , delaying hospital visits. A woman is mor e likely to die in the UK during pregnancy if she is Black\, a recent migr ant and has had inadequate pregnancy care. Cultural influences on reduced attendance for pregnancy care has been connected to Black migrant maternal deaths for twenty years. My study’s findings show that migrant Black wome n are not asked about their culture when attending for pregnancy care and they either choose to hide or reveal their culture.
\n
A persons’ culture is colourful and complex\, a n aura that is visible and invisible. The seen and unseen nature of culture makes it difficult to decipher a n individuals needs\, as culture may be practised differently depending on the environment and wider society.
\n
Culture may influence all aspects of a persons’ life including their health seeking behaviour and healthcare decisions. The missed opport unity to explore migrant womens’ cultural needs during pregnancy means tha t important aspects of their health needs are unexplored and therefore\, n ot included in their pregnancy care provision.

Background: Globally\, wat er birth is viewed with mixed opinions. While some countries endorse it fo r benefits like pain relief and a soothing birthing environment\, others a re cautious due to safety concerns and limited evidence on long-term outco mes.
\n
Aim: This study asses sed perceptions of water birth among reproductive-age women in Gombe State \, Nigeria\, focusing on perceived benefits and risks.
\n
Method: A sequential explanatory mixed-method design was used\, incorporating multistage cluster sampling. Data were co llected from 418 respondents via self-structured and online questionnaires . In-depth interviews were conducted in the qualitative phase using conven ience sampling. Ethical guidelines were strictly followed. Quantitative da ta were analyzed using descriptive statistics\, while qualitative data und erwent thematic analysis.
\n
Result: Findings indicated that 77.25% of participants held a positive vie w of water birth\, with 80.9% believing it enhances the birth experience. However\, 54.8% identified neonatal water aspiration as a potential risk. Qualitative results highlighted mixed perceptions: while some participants were positive and interested in trying water birth\, others remained skep tical\, expressing concerns about its safety.
\n
Conclusion: The study found a generally positive perce ption of water birth among reproductive-age women\, though concerns about neonatal risks such as water aspiration and maternal infection persisted.< /span>

Recommendations: Community outreach programs are essential to address misconceptions and skepticism engaging community leaders to foster positive perceptions and Support from NGOs for advocacy\, funding\, and awareness-raising to enhance water birt h’s acceptance and safety in Gombe State.

Development of midwife faculty is key for qua lity midwifery education but globally the quality and availability of prog rammes to develop midwife faculty is variable. In Bangladesh\, where inter national-standard midwifery education is still new\, faculty do not yet me et the ICM midwife teacher standard. Faculty are nurse-midwives\, though t he new generation of direct-entry midwives will soon take up positions in education. This presentation describes a peer-mentorship programme for mid wifery faculty in Bangladesh\, enabling them to teach the new curriculum t hrough non-didactic pedagogical approaches in theory and practice settings .
\n
In 2021\, twenty nationa l peer-mentors received online preparation by midwifery faculty from New Z ealand. A series of national and local stakeholder briefings took place at key points throughout the programme\, COVID-19 permitting. From 2022-2024 peer-mentors conducted in-person quarterly visits to midwifery education institutions in Bangladesh\, providing mentorship to 370 midwifery faculty and monitoring the quality of midwifery education. A digital community of practice was created to connect faculty with the peer-mentors\, with each other and with teaching resources. Baseline and endline data were collect ed using a checklist based on WHO midwifery educator competencies\, then e ntered onto a digital dashboard\; qualitative data were collected by surve y questionnaire then analysed thematically.
\n
A process evaluation of the programme in 2024 found tha t peer-mentorship had been effective in enabling faculty to implement the curriculum\, to improve the learning environment and increase students’ ex posure to midwife-led care models in practice. The programme may not be ge neralisable across all midwifery education institutions or outside of Bang ladesh.

Background: Labour hopscotch\, a visual t ool that encourages activity during pregnancy and childbirth\, has been us ed in one advanced midwife practitioner (AMP) service since 2017 and in Ir eland since 2020. A rise in caesarean births and birth weights over 4kg\, prompted the introduction of targeted weight gain as per Institute of Medi cines (IOM) guidelines in 2022. Conversations about healthy eating and exe rcise in pregnancy occur at each antenatal visit with the labour hopscotch being a visual prompt to facilitate fetal optimal positioning.
\n
Findings: Annual retrospective audit s of both interventions have demonstrated the benefits of using them. Body mass index (BMI) rates over 25 account for 61-63% of the AMP caseload. Ex cess weight gain was 24% (2023) and 29% (2024) with a resulting EMCS rate of 42% (2023) and 29% (2024) which is higher than the AMP 16% (2023) and 1 9% (2024). Before the interventions\, there was a 57% spontaneous onset of labour (SOL) rate\, with 33% birthing before 41 weeks. After the implemen tation\, there was a 72% SOL rate 2023 and 73% 2024. In 2023 92% birthed b efore 41 weeks and 83% in 2024. IOL dropped from 43% to 28% (2023) and 27% (2024)\, with emergency caesarean section (EMCS) rates following inductio n dropping from 33% to 23% (2023) and 25% (2024).
\n
Conclusions: Pregnancy is an opportunity to encour age healthy eating and exercise. These interventions should be viewed as a long-term investment by reducing maternal and childhood complications ass ociated with excess pregnancy weight gain as demonstrated in these retrosp ective audits

In Bangladesh\, as in many countries around the world\, midwives r egularly face workplace abuse\, but few incidents are reported or resolved appropriately. Most midwives/nurses and students are unaware of their rig hts to a safe workplace or learning environment and freedom from abuse\, v iolence\, discrimination or degrading treatment and may not recognise abus e when it occurs. Therefore\, in 2024\, the Directorate General of Nursing and Midwifery in Bangladesh (DGNM)\, with support from the UK and UNFPA\, developed a framework to safeguard midwives/nurses and students from work place abuse\, connect them with their workplace rights and establish a zer o-tolerance approach to harassment.
\n
A working group was formed to develop the framework\, with repre sentatives from policy level\, practice\, education\, regulatory body\, ad ministration\, professional associations and women’s groups. Expert advice was provided by a regional safeguarding specialist and the draft was vali dated at a national workshop. The framework consists of a survivor-focused standard operating procedure aligned with national laws and guidelines\, a dedicated safeguarding cell within the DGNM\, and a helpline and email a ddress for reporting abuse. There are newly-defined reporting and investig ation processes and the provision of medical\, legal and psychosocial assi stance as required. Faculty and nurse/midwife managers received training-o f-trainers and national and divisional staff\, midwives/nurses and student s received orientation.
\n
Ne xt steps in 2025 will be final approval from the Ministry\, launch and imp lementation of the framework with dissemination to all nurses and midwives \, education institutions\, students and service managers\, through the DG NM\, the Bangladesh Nursing and Midwifery Council and professional associa tions.

The provision of safe and respectful sexual and reproductive care is a basic human right and a core value of reproductive justice. A provider’s c hoice of words\, and how the words are delivered\, lays the foundation of the patient-provider encounter. Most importantly\, intentionality in choos ing respectful and inclusive language can positively influence whether pat ients feel seen and valued as human beings. Written documentation\, and ho w patients are described in electronic health records\, including historic ally stigmatized words\, can perpetuate racial disparities in healthcare d elivery. In an effort to advance health equity and improve sexual and repr oductive healthcare quality\, an extensive literature review of peer-revie wed publications in PubMed\, current textbooks\, and national guidelines o n recommended sexual and reproductive healthcare language was conducted in 2024.
\n
This presentation has 3 objectives:
\n
(1) describe the harms and opportunities in language in written and spoken form\, including the electronic health record\, pro vider-to-provider communication\, and patient-provider communication
\n
( 2) provide a review of female anatomy and procedures as gendered nomenclat ure and\;
\n
(3) review the harms of stigmatizing language and current re commendations for patient-centered and inclusive language in sexual and re productive healthcare settings and scholarly writing using sample reproduc tive healthcare vignettes.
\n
Learners will be able to revise a written vignette so that it is patient -centered and uses person-first\, anti-oppressive language.

Non-access to Perinatal ment al health services for Black African Women can lead to increased perinatal morbidity and mortality in relation to mental health. Three quarters of B lack and Brown Women’s morbidity is associated with suicide in the postnat al period\, and they are more likely to experience adverse outcomes during pregnancy and the postnatal period compared to white women (MBRRACE\, 202 3). Health inequalities within maternity services also increase the risk o f mental health challenges which is exacerbated by unemployment\, poverty and homelessness which adds to the challenges of the women accessing perin atal mental health services (Rothman et al\, 2020). There is also the asso ciated stigma within the Black African community in declaring mental healt h challenges and it being viewed as ‘shameful’ and embarrassing to admit t o feeling depression leading to alienation and preventing the women form s eeking help from health professionals (Watson et al\, 2019). The reluctanc e to access support is also compounded by negative experiences from health professionals and engagement with primary care (Edge and Mackian\, 2010). What we don’t know is how the women can be supported in accessing service s and what is required to be on place. The contribution to knowledge is ex ploring and describing attitudes to perinatal mental health\, listening to the women’s voices and understanding the service and how it can be improv ed. Methods An Exploratory Descriptive Qualitative (EDQ) study design was employed which allows for a social constructivist and interpretivist appro ach (Reid and Happell\, 2012).

C ollaboration is a cornerstone of effective\, safe\, and respectful materni ty care. The International Childbirth Initiative framework centers the rol e of midwives in the provision of respectful care and creates a space to a llow for increased understanding and promotion of midwifery across countri es where midwifery has not been integrated. The goal of the Initiative is to promote practices that allow for safe physiological birth\, including p romotion of the midwifery philosophy and access to continuous support. The need for midwives is greater than ever as globally we observe a trend of increasing maternal mortality and sharply increasing rates of cesarean bir th.

This panel will intro duce the Initiative\, with midwives speaking from participating health fac ilities in Chile\, India\, and the Philippines about their experiences of collaborating with their community\, physicians\, nurses\, administrators\ , and policy makers through the Initiative’s platform.

\n< p>Midwives play a fundamental role in the health and well-being of our communities\, but they have faced challenges such as a lack of recognition\, institutional support\, and job opportuni ties. It is essential to strengthen their role\, create jobs\, and ensure respect for their work.

Mo re than a profession\, being a midwife is a vocation that involves accompa nying mothers in crucial moments with professionalism and confidence. Howe ver\, despite their historical importance\, they often do not receive the recognition they deserve. To change this reality\, it is key to promote jo b opportunities in hospitals\, clinics\, and community health programs.
\n
The solution lies in organiz ation\, the creation of support networks\, and the pursuit of partnerships with the public and private sectors. By joining forces\, it is possible t o reduce unemployment within the midwifery community and ensure that their work is seen as indispensable to social well-being.

The path to greater recognition and job stabili ty requires commitment and collaborative work. The unity of midwives is th eir greatest strength in ensuring that their profession is valued and esse ntial in society.

Ankyloglossia\, common ly known as tongue-tie\, is an unusually short\, thickened\, or tight band of tissue tethering the tongue and restricting its movement. It is estima ted to affect 4-10% of newborns. In the newborn period\, it can be associa ted with breastfeeding difficulties\, often diagnosed when nipple pain dev elops during nursing. Longer term consequences can include eating and spee ch problems. Other oral structures can be affected by tight tissues\, such as cheeks and lips\, contributing to limited oral range of motion.
\n
Ankyloglossia occurs on a contin uum from mild to severe\, and therefore\, management options vary. Tinctur e of time and instruction on latch and positioning during breastfeeding ca n be all that is needed for the mildest cases. Other options can include p hysical therapy and massage to the oral structures of the infant. Frenulec tomy is the surgical release of the tight tissue and is accomplished with a small incision. There has been an increase in the diagnosis of ankyloglo ssia and frenulectomy\, raising concerns regarding overdiagnosis and unnec essary surgery.
\n
Midwives a re intimately involved in the initiation and maintenance of breastfeeding and the promotion of maternal and infant health\, particularly in the earl y weeks after birth. This presentation will review diagnostic criteria and the latest evidence regarding ankyloglossia and will equip the midwife wi th assessment skills and management options to offer the parents of the in fant with ankyloglossia. This is the 10th Annual Georgetown Student Cafe p resented by midwife students at Georgetown University\, Washington\, DC\, United States.

Introduction: Fear of childbirth (tocopho bia) is a significant maternal health issue that affects physical and emot ional well-being. Tocophobia can lead to complications during pregnancy an d childbirth\, requiring special attention from midwives. Factors such as maternal age\, parity\, psychological conditions\, lack of social support\ , history of abuse\, and obstetrical complications are strongly associated with this condition. This study aims to assess the prevalence of tocophob ia among pregnant women in Settat Province and identify related factors to inform midwifery practices.
\n
Methods: A quantitative study was conducted with 194 pregnant women rec ruited in Settat Province. Data collection involved a structured questionn aire comprising two sections: the first explored tocophobia-associated fac tors\, and the second evaluated childbirth fear using the EPA scale. Descr iptive statistical analysis\, mean comparisons of EPA scores\, and factor assessments were performed to identify significant correlations.
\n
Results: The average age of partici pants was 26.31 years\, with 59% multiparous and 41% primiparous women. Mu ltiparous women reported higher EPA scores (42.30) than primiparous women (40.63). Significant associations were observed between tocophobia and psy chological disorders\, experiences of violence\, and negative childbirth e xperiences (p=0.000). These findings suggest that prior trauma and psychol ogical factors are critical determinants of fear of childbirth.
\n
Conclusion: This study underscores t he necessity of evaluating tocophobia in pregnant women to improve their w ell-being and that of their children\, thereby preventing complications.

Our presentation showcases the design and implementation of a dynamic\, co-created curriculum which integrates and optimises excell ence for midwifery education. The Connected Curriculum (Fung 2017) underpi ns two distinctive UK midwifery pre-registration programmes (a three year undergraduate [BMidHons] and an integrated 4 year Masters [MMid])\, founde d on global evidence (Renfrew et al 2014) and meeting rigorous national (N ursing &\; Midwifery Council 2019) and global (International Confederation of Midwives 2021) midwifery education standards.

Fung’s (2017) Connected Curriculum is values based . Learning through research and inquiry forms the central core\, linked to six dimensions which enable connectivity to midwifery as follows < span style='font-weight: 400\;'>a through line of research activity\, supp orting students to:

Mothers and Babies of African Descent have some of the poorest outcomes of the childbearing continuum irrespective of their gross national income (GNI) per capita\, whether low\, medium or high income countries.\n
The establishment of the Permanent Forum on People of African Descent at the United Nations (Geneva) in Dece mber 2022 began the journey to create a dedicated forum to focus on midwiv es and mothers who are on the ground delivering and receiving maternity c are within those communities. Moth er and Midwives Across the Diaspora has made intervention at each Permanen t Forum Session to support this work.

The aim of the presentation is to share the journey to date of this unique network\, to refocus on the value of midwifery to amelioratin g mortality and morbidity of Black mothers and babies worldwide. It will describe the approach being used to garner the knowledge and skills of midwives across the world\, utilising every means of supporting professional and vocational approaches to achiev e its aims. It will share outcomes and achievements. The presentation will welcome ongoing support of deleg ates in the furthering of good global midwifery relationships.

\n< p>Introduction: Integrating artificial in telligence (AI) into healthcare holds transformative potential for midwife ry\, enhancing prenatal care\, labour and delivery outcomes\, and postnata l support. This systematic review evaluates how AI technologies can improv e decision-making\, client outcomes\, and personalized care\, highlighting the urgency for midwifery professionals to adapt to this emerging innovat ion.
\n
Methods: Following a PROSPERO-registered protocol and PRISMA guidelines\, this review investiga ted the impact of AI interventions in midwifery. A comprehensive search of electronic databases (PubMed\, Cochrane Library\, EMBASE) and grey litera ture identified studies focusing on AI applications in women’s health\, in cluding prenatal\, labour\, and postnatal care. Data extraction captured k ey insights into the implementation and impact of AI. The PRISMA flow diag ram documented the study selection process.
\n
Results: The review explored AI applications across medi cal imaging\, diagnostics\, predictive analytics\, personalized medicine\, natural language processing\, robotics\, virtual health assistants\, and genomics in midwifery. AI technologies improved diagnostic accuracy\, pati ent monitoring\, risk assessment\, treatment personalization\, and expedit ed drug discovery. Moreover\, by automating routine tasks and reducing adm inistrative burdens\, AI supports midwives in focusing more on client care and their well-being. These innovations streamline clinical workflows and enhance patient outcomes\, demonstrating AIs transformative potential in midwifery.

Conclusions: AI integration in midwifery is an emerging reality with profound implication s for practice and care. Preparedness through training\, professional deve lopment\, and supportive regulations is essential to ensure ethical and ef fective adoption. Future research should address integration guidelines\, challenges\, benefits\, and long-term impacts\, ensuring AI complements mi dwifery’s core values while advancing client care.

This presentation will explain how sustained umbilical co rd circulation from placental transfusion after birth facilitates a large auto-transfusion from the placenta to the infant that holds potential for saving babies lives. This transfer of blood provides an innate force withi n the newborn’s body enhancing blood flow and perfusion\, essential for no rmal growth\, development\, and regeneration. High progesterone levels\, p rime the body to receive blood at this time. Enhanced blood volume causes high pulmonary artery pressure for approximately the first 10 post-birth h ours likely driving maximum perfusion throughout the whole body. The enhan ced blood volume floods the newborn’s brainstem and other organs\, prevent s hypovolemia as well as subtle or overt ischemia\, and helps the newborn regain homeostasis after the stress of birth.

This presentation provides evidence for each facet\, explains how they work together to ensure newborn health\, and will discus s the high cost of immediate (ICC) or early cord clamping (ECC).

The Birth Future Intern ational Project explores innovative future scenarios for birth care\, grou nded in developments shaped by birthing communities and midwives as key ag ents of change. This study employs a qualitative method approach\, integra ting traditional participatory research methodologies with innovative\, ar ts-based practices to guide participants through immersive processes of ex ploration and co-creation. Our cross-cultural sampling includes midwives f rom the Netherlands and Peru 90 \, alongside service users from India\, offering diverse perspectives on the evolving dynamics of birth care.
\n
Our data analysis is based on action research analysis. Preliminary data analysis has informed the devel opment of a zine\, which synthesizes participant contributions into an acc essible\, creative format. This zine unveils a transformative vision for b irth care\, such as different ways of developing perinatal technology (tec hnology for autonomy\, rather than control)\, the role of the community in which birth (care) is embedded\, addressing systemic injustice and inequa lity in global north and south\, midwives as educated birth advocates/port als for bridging new ways of (birth) care\, those articulated through crit ical reflections on participants’ lived experiences and current practices. Participants then identified innovative\, community-centered care models that emphasize equity\, cultural responsiveness\, and inclusivity. Further more\, the preliminary findings underscore the potential of participatory\ , arts-based research to amplify underrepresented voices and foster critic al discourse on the future of maternal and newborn care.

C’est le cas pour l’insc ription de notre activité professionnelle au patrimoine immatériel UNESCO où\, dans l’annonce en français\, le mot sage-femme est absent du titre. C omment sont présentées les sages-femmes ailleurs ? Neuf documents ont été repérés qui ont pour titre l’ar t\, les soins\, la pratique\, les sciences ou la profession de sage-femme< /span>. Les résumés de ces documents seront présentés\, ainsi que l’analyse réalisée\, basée sur les critères d e soins centrés sur le patient (Ry croft-Maloine\, 2004). Les résulta ts montrent que toutes ces formulations sont polysémiques et qu’elles n’on t pas exactement la même portée. P ar contre\, toutes présentent haut et fort les sages-femmes et ce qui les caractérise. Si UNESCO avait titré « Les soins de sage-femme : connaissances\, savoir-faire et pratiques »\, les sages-femmes seraient visibles partout dans le monde francophone.

The theme of the intervention is motivated by a regrettable problem of visibility of midwives in certain publications. This is the case for the inclusion of our professional activity in UNESCO’ s intangible heritage list\, where\, in the French announcement\, the word sage-femme is absent from the title. How are midwives presen ted elsewhere? Nine documents have been identified that deal with < i>the art\, care\, practice\, science or profession of midwifery. Summa ries of these documents will be presented\, along with the analysis carrie d out\, based on the criteria of patient-centred care (Rycroft-Maloine\, 2004). The results show that all these formulations are polysemous an d do not have exactly the same scope. However\, they all make a strong cas e for midwives and what characterises them. If UNESCO had published the ti tle « Les soins de sage-femme: connaissances\, savoir-faire et pratiques » (‘Midwifery: knowledge\, skills and practices’)\, midwives w ould be visible throughout the French-speaking world.
