15 Insights into Postpartum Depression in Indonesian Mothers: Mixed Method Approach

Background: Postpartum depression affects maternal well-being and infant outcomes, yet its interplay with emotions and support remains underexplored in Indonesian contexts. The prevalence of postpartum depression in Indonesia ranged from 2.3% to 22%. This mixed-methods study aims to identify depression prevalence and qualitative themes during postpartum periods.

 Methods: A mixed-methods design was employed with 54 Indonesian mothers from several hospitals using descriptive univariate quantitative EPDS screening and qualitative analysis of written reflective statements. Thematic analysis was identified from reflections of the mother related to their emotions and concerns within 6 weeks after giving birth. 

 Results: The mothers were mostly aged 19-32 years (80%); 48% primiparous and 52% multiparous; 43% vaginal delivery, and 57% with Caesarean Section. EPDS (α = .863) revealed 35% probable depression, 26% possible mild depression, and 39% no depression. The written emotions showed that mostly mothers have experienced positive emotion (57%) and negative emotion (43%) during the postpartum period. Four themes emerged for concerns related to partner and family support, varying from unavailable to available support; perinatal mental health concerns, such as experiencing anxiety, postpartum blues, and depression; breastfeeding concerns; and role adjustment as a mother of multiple children. 

 Conclusion: High probable depression rates underscore the need for routine screening for perinatal mental health and targeted support in maternity care. Healthcare professionals can provide support and education to mothers and their families to navigate the postpartum period better.
