07 Strengthening Culturally Responsive Midwifery to Improve Nutrition in the First 1,000 Days for Vulnerable Mothers and Infants in Indonesia

Abstract:- Vulnerable mothers and infants, such as those in urban-poor settlements, street-connected families, and adolescent mothers, face significant barriers to equitable, rights-based maternal and newborn care. In Indonesia, cultural norms surrounding feeding, caregiving, and health-seeking behaviors significantly influence nutrition during the first 1,000 days, yet facility-based services often fail to reach these groups. This study examined a culturally responsive, multisector, midwifery-led strategy to strengthen maternal infant nutrition and early disease-prevention behaviors among vulnerable populations.

 Using a mixed-methods design, the study integrated quantitative program data, structured observations, and thematic feedback from mothers, caregivers, cadres, and midwives. Interventions included tailored nutrition counseling, culturally adapted responsive feeding demonstrations, mental health and infection-risk screening, early stimulation, and home-based growth monitoring.

 Quantitative results showed marked improvements: maternal nutrition knowledge increased by 35–40%, confidence in exclusive breastfeeding rose by 40%, and responsive feeding practices improved. Reports of harmful food taboos dropped by over 30%, accompanied by greater involvement of grandmothers and fathers. Qualitative findings highlighted improved maternal autonomy, stronger trust in midwives, and enhanced dignity in care. Midwives also reported better detection of growth faltering and infectious risks, as well as stronger referral pathways.

 The findings show that culturally grounded, midwife-led, multisector approaches can advance equitable, respectful, rights-based maternal care for vulnerable groups and provide a scalable model for LMICs.
